I saw this at the 2003 National Sports Collectors Convention in  Atlantic City in the Hunt Auctions booth. I believe it's the only example I've seen as I can't recall another, making it pretty rare. Hunt sold it in their February 2004 auction, Lot 271, for $650.00. In the catalog description it says it was the only one they had seen. While preparing this feature I found a photo I had of an actual package of Pinch Hit, see below:
